  • 年度更新のメールを参照しないと、正しく記入できない

• Registration form を埋める．
  • Team Leader (石野 or 隅田) のサインをもらう．
  • Average presence at CERN の欄は最初に登録する際には 20%にしておく．(50% 以上だと他の書類も必要になる)
  • Expected overall period　→　修士課程修了までの日付を書いておく．
  • Internal address →　Building: 188 Floor: 5 Office: 006 Tel 74524
  • Home Institution Declaration
  • 下の欄には物２主任のサイン、判子をもらう．主任の英語訳の例： “The chairperson of the department of physics”
  • Stamp の欄には事務室にある物２教室のスタンプを押してもらう．
